    By using WikiForms, you agree to these terms. WikiForms is a volunteer-run, open-source tool hosted on Wikimedia Toolforge.
  

1. Acceptance of Terms

By accessing or using WikiForms ("the Service"), you agree to be bound by these Terms of Use and our Privacy Policy. If you do not agree, please do not use the Service.

2. Description of Service

WikiForms is a free, open-source form and quiz builder designed for the Wikimedia ecosystem. It is hosted on Wikimedia Toolforge and is subject to Wikimedia Cloud Services Terms of Use.

3. Eligibility

To create forms or quizzes, you must have a valid Wikimedia account. Anyone may respond to a published form without an account.

4. Acceptable Use

You agree NOT to use WikiForms to:

Collect personally identifiable information (PII) including names, addresses, phone numbers, or financial information
Collect sensitive personal data including health, political, or religious information
Harass, deceive, or harm other users
Violate any applicable laws or regulations
Circumvent security measures or attempt unauthorized access
Use the service for commercial purposes or advertising
Create spam or distribute malicious content

5. Data and Content

You retain ownership of the content you create on WikiForms. By submitting content, you grant WikiForms a limited license to store and display that content solely for the purpose of operating the Service.

Form responses submitted by users are visible only to the form owner and explicitly authorized collaborators. You are responsible for ensuring that the data you collect through your forms complies with applicable privacy laws and Wikimedia policies.

6. Prohibited Data Collection

In compliance with Toolforge Terms of Use Section 7.2, WikiForms must not be used to collect personally identifiable information. Form creators are solely responsible for ensuring their forms comply with this requirement.

7. Service Availability

WikiForms is provided "as is" without warranty of any kind. As a volunteer-run project on Toolforge infrastructure, we cannot guarantee uninterrupted availability. The service may be modified, suspended, or discontinued at any time.

8. Limitation of Liability

WikiForms and its contributors shall not be liable for any direct, indirect, incidental, or consequential damages arising from your use of the Service. Use of WikiForms is at your own risk.

9. Intellectual Property

The WikiForms source code is licensed under the MIT License. Documentation and non-code assets are available under CC BY-SA 4.0.

WikiForms is not affiliated with or endorsed by the Wikimedia Foundation. "Wikipedia" and "Wikimedia" are trademarks of the Wikimedia Foundation.

10. Changes to Terms

We may update these Terms of Use at any time. Continued use of the Service after changes constitutes acceptance of the new terms. The "Last updated" date at the top of this page will reflect any changes.

11. Governing Policy

These Terms are supplemental to and governed by the Wikimedia Cloud Services Terms of Use and the Wikimedia Foundation Privacy Policy.
